FAMILY SUPPORT PROVIDER

KNOWLEDGE, SKILLS, AND ABILITIES:
- High School Diploma or equivalent.
- Experience in seeking and accessing state and community resources and a willingness to help families access resources.
- Experience raising a child with behavioral/emotional issues, accessing services for children with behavioral/emotional issues and navigating the child serving system.
JOB DUTIES AND RESPONSIBILITIES:
- Assist families in developing and recruiting informal supports to be a part of the Family Team meetings.
- Complete (or co-complete) Strengths Needs Cultural Discovery (SNCD) assessments, Functional Assessments/Crisis Plans, Safety Plans, and Care Plans/Updates.
- Provide support for families with the eventual goal of the family utilizing their natural supports.
- Facilitate or co-facilitate Family Team meetings and ensure informal supports are included and part of the team.
- Implement the Do For, Do With, and Cheer On model for families.
- Maintain close communication with the Integrated Team Manager and other clinical and administrative staff in order to ensure the highest quality services are delivered for youth and families.
- Complete flex-fund requests when necessary.
- Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs.
- Personality and mature presentation of oneself, demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.

KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge of the principles and applications of psychological theories and treatment models with respect to group and individual behavior; of mental illness, its symptoms, treatment, and social implications in relation to individuals/families.
- Skill in assessing and diagnosing for the development of comprehensive treatment plans.
- Skills for evaluating a wide variety of clinical data for use in carrying out courses of treatment.
- Ability to communicate clearly in both oral and written form.
- Knowledge of the principles and practices of social work and/or psychology as it relates to current social and economic problems to better serve individuals/families.
- Knowledge of personality development and structure.
- Knowledge of general age groups and other special populations, as well as core competencies in relation to individuals with co-occurring disorders.
- Ability to correctly interpret and implement agency policies and procedures.
- Knowledge of appropriate management and supervisory principles and the ability to implement accordingly.
- Ability to evaluate situations and adopt an effective course of action, delegate responsibilities, organize, and present facts effectively.
- Ability to work well under pressure, meeting deadlines with efficiency and consistency.
- Ability to interface with individuals/families, administration, clinical staff, support staff, and the public in a clear, polite, professional, and respectful manner.
JOB DUTIES AND RESPONSIBILITIES:
This position is responsible and held accountable for the following duties:
- Coordinate, synchronize, and manage the crisis care and stabilization of clients on the crisis unit.
- Work in partnership with the Crisis Services Coordinator to ensure continuity across shifts and units including, ensuring protocols/policies/procedures are being implemented, trained, and followed up on with consistency.
- Must meet agency standards of service hours, productivity, and fiscal responsibility.
- Responds to emergency crisis calls from or with regard to clients, provides crisis intervention, counseling, or referral to appropriate agency based on client needs.
- Ensure successful and timely implementation of special projects, new services, etc. as required.
- Provide daily oversight to all assigned team members including coordination, planning and delegation of work to assist individuals/families. Team members may include a combination of psych tech, peer recovery support specialist, and nursing staff.
- Review timecards for accuracy, adjusting time entries as needed and final approving all timekeeping records for assigned team. Will work in conjunction with Crisis Services Coordinator to approve time off requests.
- Provide guidance and support to team members to resolve employee relations issues working with management team and Human Resources.
- Ensure implementation of all crisis intervention plans.
- Screening and crisis intervention as directed.
- Assessment of individuals to determine client’s eligibility for admission to the crisis unit and continued assessments to obtain information relevant to presenting problem, need for emergent services, and referral to appropriate clinical services.
- Leads participation in diagnostic and evaluative staff meetings to discuss the diagnosis, etiology, treatment and disposition of patients.
- Provides diagnostic impression and crisis intervention planning.
